I, the undersigned, am the parent or legal guardian of the applicant listed above. I hereby give my full consent for their participation in the "Spend a Day With J" mentoring experience with The Spratling Family Foundation. I understand the nature of the activities involved and give permission for:

• My child to travel to and attend this program.

• Photographs/videos to be taken for educational and promotional purposes.

• Emergency medical treatment to be administered in case of illness or injury.
